BCBS Medical Plan
HSA qualified High Deductible Health Plan with a $3,000 individual deductible and $3,000 individual annual out-of-pocket maximum. Once the individual deductible is met, there is no charge for covered services for the remainder of the calendar year. Expansion will match HSA up to $500 a year.
Principal Dental Plan
Calendar Year Maximum benefit is $1,500 with a $50 deductible (waived for Preventative services) Preventative services are covered at 100%, Basic covered at 80% and Major Services covered at 50%.
Principal Vision Plan
$150 Frames benefit every 12 months, Exams $10 copay and $25 materials copy for lenses. $150 allowance for elective contacts in lieu of glasses.
Expansion is covering 70% of the premiums for all levels of coverage for medical, dental, and vision.
Principal 75K Term Life/AD&D
All eligible full-time employees receive a $75K Life/AD&D plan paid for by Expansion Ammunition.
Principal Voluntary Life/AD&D
All eligible full-time employees can purchase Voluntary Life/AD&D Insurance for themselves and their dependents. Employees choose benefit amount between $10,000 and $300,000, spouse $5,000 up to $100,00 and children $2,000, $5,000, or $10,000.
Guaranteed Issue amounts are Employee $100,000; Spouse $20,000 and children $10,000. Should you elect an amount greater than the guaranteed issue amount, a health questionnaire will be required for approval.
Expansion will provide several “work perks” on top of benefits such as: breakfast and lunch for full-time employees each workday, flexible time-off (within reason) for family, children & church events, automobile and home loan assistance, free membership to Planet Fitness, free weekly car wash, toast masters speaking and communications training, and Dave Ramsey Financial University. In the future we plan to implement an employee discount for products we manufacture.
